29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:      a) 405 Route 22 East, Whitehouse Station, NJ 08889. The employer allowed his employees to operate a Bostitch pneumatic nail gun while installing new roofing materials when the employees were no wearing any eye and or face protection.       Condition occurred on or about 08/16/14.

29 CFR 1926.501(b)(1): Employee on a walking/working surface having an unprotected side or edge which is six feet or more above a lower level were not protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system:    a) 405 Route 22 East, Whitehouse Station, NJ 08889. The employer allowed his employees to work on and from the roof of a commercial building installing new roofing materials with no fall protection systems in place.      Condition occurred on or about 08/16/14.

29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist the employees in mounting and dismounting the ladder:      a) 405 Route 22 East, Whitehouse Station, NJ 08889. The employer allowed his employees to use a 20 foot portable extension ladder to access the roof when the ladder did not extend three feet above the landing.       Condition occurred on or about 08/16/14.
