C.W. Resources, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1910.212(b), which was contained in OSHA Inspection 911578, Citation No. 01, Item No. 003, and was affirmed as a final order December 9, 2013, with respect to a workplace located at 250 Aiken Street, Building 26, Shaw Air Force Base, SC  29152.

29 CFR 1904.7(b)(3): The employer did not correctly classify the following work-related injuries or illnesses on the OSHA Form 300 or equivalent:  Located at Building 26, Shaw Air Force Base, SC: On or about 8/19/14, the employer did not enter correctly the following work-related injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2014:  (a) On or about 7/14/14, a mower operator sustained herniation of a vertebrae disk in his back while operating a tractor mower, resulting in approximately 3 weeks of restricted work activity and 1 week of lost time. The employer failed to record this case on the OSHA Log.    Abatement Certification and Documentation is required

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:   (a) As of August 19, 2014, the employer had not trained employees on the new chemical container label elements and the new format and types of information on each section of safety data sheets (SDSs) . Employees were potentially exposed to such hazardous chemicals as gasoline, diesel fuel, welding gases, mineral spirits, grease, and engine oil.    ABATEMENT CERTIFICATION IS REQUIRED
