29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Equipment located throughout the production facility; On or about August 6, 2014 and at times prior thereto, the employer exposed employees to potential hazardous energies in that employees were permitted to perform maintenance and servicing on equipment in the facility (including but not limited to the #1 continuous oven and FM-003 line) without having developed written machine specific lock-out tag-out procedures for equipment with more than one energy source.
