29 CFR 1903.19(d)(1):  The employer did not submit tot the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:  On or about October 21, 2014, the employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1 Item 1a-c, Citation 2 Item 1a-b, and Citation 3 Item 1a-c for hazards associated with the following:  (a)  Employee exposure to falls without adequate fall protection, specifically utilizing fall monitor;  (b)  Employee exposure to falls without adequate fall protection, specifically fall arrest systems or other means of fall protection; and,  (c)  Employee exposure to hazardous chemicals and lack of a hazard communication program, safety data sheets, and associated required training.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.    On or about August 23, 2014, employees were exposed to fall hazards while working on a low sloped roof in areas where the warning line system had been removed. Adequate means of protection from falls were not in place.   R & R Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(a)(1) which was contained in OSHA inspection number 944775, citation number 1, item number 1a, and was affirmed as a final order on December 7, 2013, with respect to a workplace located at 6034 Lake Road, North Ridgeville, OH.  R & R Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(a)(1) which was contained in OSHA inspection number 903518 citation number 1, item number 1a, and was affirmed as a final order on May 28, 2013 with respect to a workplace located at 1850 East Erie Avenue, Lorain, OH.
