29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:  (a) EDC, High Rise, Maintenance Area - An employee worked replacing a wheel on Cleco crane 0265.  The employee had opened the disconnects to isolate this crane from the AC and DC electric; however, the employee did not apply lockout/tagout devices to the energy isolating devices, exposing the employee to injurious contact with electric current, caught-in, struck-by and other hazards resulting from unexpected energization, on or about August 28, 2014  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft ends did not present a smooth edge and end and projected more than one-half the diameter of the shaft:  (a) EDC, Active Item 2 Area - The P-202 conveyor section, EJON 4D9314, had an unguarded shaft end, 4 feet 4.5 inches above the floor, that did not present a smooth edge and end in that the shaft end had a key way and the rotating collar had two set screws.  In addition, the P-202 conveyor section, EJON 4D9313, had an unguarded shaft end, 2 feet 4.5 inches above the floor, that did not present a smooth edge and end and projected more than one-half the diameter in that this shaft end had a diameter of 1 inch and projected 5/8 inch, had a key way, and the rotating collar had two set screws.  Both of these shaft ends exposed employees to a caught in rotating parts hazard, on or about August 27, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.305(b)(1): Unused openings in boxes, cabinets, or fittings were not effectively closed:  (a) EDC, Active Item 2 Area - The junction box, 5 feet from conveyor section P-202, EJON 4D9313, had a knock out missing on the right side of the box, exposing employees to the hazard of contacting electrical current, on or about August 27, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.334(a)(2)(ii): When there was a defect or evidence of damage that could expose an employee to injury, the defective or damaged item was not removed from service until the repairs and tests necessary to render the electric equipment safe had been made:  (a) EDC, Active Item 2 Area - On the Patton pedestal fan, model 30ACP-U, serial number 98-30P-0000054, the outer jacket of the electrical cord, which runs through the post of the fan, had separated at the socket end of the cord, exposing employees to the hazard of contact with electric current, on or about August 27, 2014.  No abatement certification or documentation required.

1910.1200(h)(3)(iv): The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  (a) DLA Distribution Susquehanna Pennsylvania, 2001 Mission Drive, New Cumberland, Pennsylvania - Employees used or worked around hazardous chemicals, including but not limited to Sprayon LU 202 Moly Chain Lubricant and MAPP gas, and the employer did not provide all employees with training on the revised hazard communication standard (HCS 2012) label elements and the new format and type of information on each section of the safety data sheets (SDSs) to ensure that when employees begin to see HCS 2012 compliant labels and SDSs in their workplace, employees will understand how to use the labels and SDSs and access the information effectively, on or about August 27, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
