29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening: On or about August 28, 2014 at the above addressed jobsite, an employee was standing in the bucket of a Case 570 LXT front end loader that was being used to elevate the employee, the company did not ensure the platform was adequate to lift the employee in, exposing the employee to death or permanent disability.
