29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions:    a. On or about January 29, 2016, employees working in an excavation 5 feet 11 inches deep were exposed to cave-in hazards at 2204 6th Avenue in Rock Island, Illinois and the employer did not inspect the excavation before a worker entered the excavation.      b. On or about January 29, 2016, employees working in an excavation up to 6 feet 4 inches deep were exposed to cave-in hazards at 2210 6th Avenue in Rock Island, Illinois and the employer did not inspect the excavation before a worker entered the excavation.      c. On or about January 29, 2016, employees working in an excavation up to 8 feet 4 inches deep were exposed to cave-in hazards at 2214 6th Avenue in Rock Island, Illinois and the employer did not inspect the excavation before a worker entered the excavation.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) sloping and benching systems, or paragraph (c) support systems, shield systems, and other protective systems:    a. On or about January 29, 2016, employees working in an excavation 5 feet 11 inches deep were exposed to cave-in hazards at 2204 6th Avenue in Rock Island, Illinois and the employer did not ensure the excavation was sloped, shored, or braced in accordance with the standard.       b. On or about January 29, 2016, employees working in an excavation up to 6 feet 4 inches deep were exposed to cave-in hazards at 2210 6th Avenue in Rock Island, Illinois and the employer did not ensure the excavation was sloped, shored, or braced in accordance with the standard.       c. On or about January 29, 2016, employees working in an excavation up to 8 feet 4 inches deep were exposed to cave-in hazards at 2214 6th Avenue in Rock Island, Illinois and the employer did not ensure the excavation was sloped, shored, or braced in accordance with the standard.     BRANDT CONSTRUCTION CO was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard: 29 CFR 1926.652(A)(1) which was contained in OSHA inspection number 1065292, citation number 1,  item number  1, AND WAS AFFIRMED AS A FINAL ORDER ON 06/30/2015 with respect to a workplace located at 11 ½ street, 6TH avenue, ROCK ISLAND ILLINOIS, 61201.
