29 CFR 1910.146(d)(3): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations:  a.  1510 Coronet Drive, Dalton GA - The company did not develop and implement a means or procedure, to control all potential hazards prior to entering the permit required confined space in mixing tank D, thereby exposing the employee to hazards such as but not limited to falls, noise, potential hazardous energy release, moving mechanical parts, engulfment and drowning.
