29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked between the front uprights and the guardrail supports: a) Two planks were missing on the scaffold platform, exposing employees to fall hazard; on the tubular welded frame scaffold surrounding the top of the building; 20th floor. Location: 812 5th Ave, New York, N.Y. On or about 8/6/2014 NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.

29 CFR 1926.451(b)(5)(i): Each end of a scaffold platform 10 feet or less in length shall not extend over its support more than 12 inches (30 cm) unless the platform is designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping, or has guardrails which block employee access to the cantilevered end: a) Scaffold planks were extended 30 inches beyond the support point of the scaffold frame, causing the plank to up-end when employee stepped on the end of the plank. Location: 812 5th Ave, New York, N.Y.
