29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:     a) Facility, Chemical Storage Trailer- An eye wash providing at least 15 minutes of a continuous flow was not provided for use by employees who work with corrosive products including but not limited to phosphoric acid F.G. , Oakite Fisan Oxysanitizer, and Vigilquat, observed on or about 9/30/14.    b) Facility, Roof Access Room- An eye wash providing at least 15 minutes of a continuous flow was not provided for use by employees who work with corrosive products including but not limited to Vigilquat and Sodium Hypochlorite, observed on or about 9/30/14.    NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:    a) Facility: Employees working with and around products including but not limited to, phosphoric acid F.G. , Oakite Fisan Oxysanitizer, and Vigilquat were  not provided with training on the revised HCS label elements and the new format and types of information on each section of the safety data sheets (SDSs), occurred on or about 8/27/14.
