29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a) Raw Slice 2: The Anco slicer was not provided with a guard which would prevent an employee from placing his hands in the danger zone during the operating cycle. On 7/29/15 a temporary employee was injured when his hand came into contact with a moving blade. b) Mold Room: The grinder was not provided with a guard which would prevent an employee from placing his hands in the danger zone during the operating cycle. On 8/31/15 an employee was injured when his right index finger came into contact with a moving blade. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.
