29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:    a)  Raw Slice 2: The Anco slicer was not provided with a guard which would prevent an employee from placing his hands in the danger zone during the operating cycle. On 7/29/15 a temporary employee was injured when his hand came into contact with a moving blade.     b)  Mold Room: The grinder was not provided with a guard which would prevent an employee from placing his hands in the danger zone during the operating cycle. On 8/31/15 an employee was injured when his right index finger came into contact with a moving blade.    NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.

29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) projected more than one half the diameter of the shaft:    a) Facility, Conveyor to grinder: A rotating shaft approximately 1 inch in diameter projected out approximately 1 inch, on or about 9/25/15.    b) Facility, MPO, Oven Conveyor to CPLO#2: A rotating shaft approximately 1 1/4 inches in diameter projected out approximately 1 inch, on or about 9/25/15.     c) Facility, CPLO#2: A rotating shaft approximately 1 1/4 inches in diameter projected out approximately 2 1/4 inches, on or about 9/25/15.    d) Facility, CPLO#2, Inspection Station: A rotating shaft approximately 1 1/4 inches in diameter projected out approximately 1 1/4 inches, on or about 9/25/15. 29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours:    a) Located at Allied Specialty Foods, Vineland NJ: On or about 7/29/15, an employee received an injury to his left hand resulting in an amputation. The employer did not report the amputation to OSHA within 24 hours.    b) Located at Allied Specialty Foods, Vineland NJ: On or about 8/31/15, an employee received an injury to his finger resulting in a partial amputation. The employer did not report the amputation to OSHA within 24 hours.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  (a) Facility: Employees working with and around sanitizing products including but not limited to Vigilquat and sodium hypochlorite, were not informed of the hazards associated with hoes products, on or about 8/20/15.  NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.
