29 CFR 1910.303(b)(1):  Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:  a)  Next to Chrome bath, On or about August 07, 2014, employees were exposed to an electrical hazard while working near the Dynapower Power Conversion System which had a flexible conduit, which supplied electricity to the unit, which was disconnected from the connector, thus exposing the internal wiring within the conduit.  b)  Above the bench, Polishing cleaning area, On or about August 07, 2014, employees were exposed to an electrical hazard while working near the Quad 120 Volt AC Receptacle which had a flexible conduit which was not continuous thus exposing the internal wiring within the conduit.  c)  Nickel bath area, On or about August 07, 2014, employees were exposed to an electrical hazard while working near the Nickel bath tank, which had an electrical junction not contained within a junction box, thus exposing employees to the 120 Volt AC wiring nut junction.  d)  Nickel bath area, On or about August 07, 2014, employees were exposed to an electrical hazard while working near the Nickel bath tank, which had a flexible conduit, supplying electricity to the bath heater, that was disconnected from the connector thus exposing the internal 120 Volt AC wiring within the conduit.

29 CFR 1910.305(b)(1):  Unused openings in boxes, cabinets, or fittings were not effectively closed:  a)  Stationary Polisher #1 (Left), On or about August 07, 2014, employees were using the polisher while the electrical cabinet had an approximately 1" x 5" rectangular opening exposing employees to the internal 120 volt AC wiring.  b)  Next to Chrome bath, On or about August 07, 2014, employees were exposed to an electrical hazard while working near the DC Power Supply which had missing knockouts thus exposing the power supplies internal wiring.  c)  Above the bench, Polishing cleaning area, On or about August 07, 2014, employees were exposed to an electrical hazard while working near the Quad 120 Volt AC Receptacle which had a missing knockout.  d)  Chemical bath area, break room wall, , On or about August 07, 2014, employees were exposed to an electrical hazard while working near the rinse tank, which had an electrical disconnect providing electricity that had a missing knockout thus exposing employees to the disconnects internal 240 Volt AC wiring.  e)  Chemical bath area, break room wall, , On or about August 07, 2014, employees were exposed to an electrical hazard while working near the circuit breaker panel "D" which had a missing circuit breaker knockout, thus exposing employees to the 240 Volt AC bus bar.
