29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. (a). At the Burger King renovation site in Apopka where stucco finishing was being performed, the supported scaffold being used by employees did not have a completed guardrail system at its third level exposing the employees to a 14-foot fall hazard, on or about September 3, 2014.

29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toe boards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contained or deflected the falling objects: (a). At the Burger King job site no toe boards were installed on the second level of the scaffold platform to provide protection to the employees working below from being struck by falling material, on or about September 3, 2014.
