29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a. United Community Bank, 3510 Henderson Mill Road, Chamblee, GA (Jobsite) - Subcontractor employees were exposed to fall hazard. Subcontractor roofer employees were not utilizing a means of fall protection and/or tied off to an anchorage point while walking and working approximately 20 feet above ground level on the elevated roof with a 10:12 slope pitch. The subcontractor employees were installing new roof materials such as, felt paper and roof shingles on the elevated roof, on or about 09/03/14.

29 CFR 1926.1053(b)(4): Ladders were not used for the purpose for which they were designed: a. United Community Bank, 3510 Henderson Mill Road, Chamblee, GA - Subcontractor employees were exposed to fall hazard. Subcontractor employees took apart a 30' aluminum portable extension ladder and use one section of the ladder 15' as a crawling chicken board ladder which was used as steps to gain access to an upper level roof area, on or about 09/03/14.
