29 CFR 1910.184(e)(3)(i): Thorough periodic inspections of alloy steel chain slings in use were not made on a regular basis:  Throughout the main shop area the employer had not ensured that the alloy steel chain slings were subjected to the required thorough inspection at least every 12 months as required. Employees were exposed to the hazard of dropped loads and/or struck-by hazards.

29 CFR 1910.305(b)(1)(ii): Unused openings in cabinets, boxes, and fittings were not effectively closed.  (a) In the shop there was an unused open hole in the 480 volt receptacle box providing power to the Miller Welder. Employees were exposed to a shock hazard.   (b) In the shop in the lathe area there was an unused open hole in the electrical box feeding power to a thermostat. Employees were exposed to a shock hazard.   (c) On the west side of the blast booth there was an unused open hole in a switch box. Employees were exposed to a shock hazard.

29 CFR 1910.303(g)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by approved cabinets or other forms of approved enclosures, or other means listed under this provision:  On the post next to the aluminum side rail jig there was a junction box that no longer had the cover firmly attached and covering the energized electrical conductors located within. Employees were exposed to a shock hazard.
