29 CFR 1910.119(j)(2): The employer did not establish or implement written procedures to maintain the on-going integrity of process equipment:    a) 125 Factory Ln, Middlesex, NJ site -  Process area:       Employer failed to establish written procedures  for inspecting and testing pressure relief devices such as but not limited to pressure relief valves and conservation vents located on the Still #5 - fractional distillation column and the molecular sieve. 29 CFR 1910.119(j)(4)(iii): The frequency of inspections and tests of process equipment was not consistent with applicable manufacturers' recommendations and good engineering practices, and more frequently if determined to be necessary by prior operating experience:    a) 125 Factory Ln, Middlesex, NJ site -  Process area:       Employer failed to establish a definite time interval between inspections or tests for pressure relief valves at the worksite based on operating experiences, good engineering practices and manufacturers' recommendations.    Condition noted on or about 02/05/15.     RAGAGEP such as but not limited to API 510 which gives guidance as to establishing and implementing defined inspection intervals was not followed:    6.6.2 Testing and Inspection Intervals  6.6.2.2 Unless documented experience and/or a RBI assessment indicates that a longer interval is acceptable, test and inspection intervals for pressure-relieving devices in typical process services should not exceed:  a. Five years for typical process services; and,  b. Ten years for clean (non-fouling) and noncorrosive services.

29 CFR 1910.219(c)(2): All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform, excepting runways used exclusively for oiling, or running adjustments, were not protected by a stationary casing enclosing shafting completely:    125 Factory Ln, Middlesex, NJ site -  Process area: Employer did not provide stationary casing on horizontal shafts.    a) Still #2 - Moving parts of the horizontal shafting of the motors which control the agitators inside of Stills #1 not adequately guarded.    b) Still #1 - Rotating horizontal shaft on the ground level motor was not adequately guarded as the metal guard was damaged.    Condition noted on or about 09/04/2014.
