29 CFR 1910.119(f)(1)(iii)(B): The employer failed to ensure that the Process Safety Management Standard Operating Procedures included safety and health consideration precautions necessary to prevent exposure, including engineering controls, administrative controls, and personal protective equipment. The operating procedures for Unit 1 and Unit 2 would not prevent exposure to flash fires in that chemical resistant coveralls listed as personal protective equipment were not flame resistant.
