While the cover is not in place, the pit or trap opening was not constantly attended by someone or was not protected on all exposed sides by removable standard railings:  a)  Throughout the wash room area - Employees were working and walking near floor pits, measuring approximately 15-18 inches wide, 18-70 inches in depth, and various lengths (up to 8-10 feet and no less than 1 foot), without being guarded by covers of standard strength and construction.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:    a)  At the wash room area - Employees were using a flexible cord (extension cord) to power a 36-inch portable circular fan, where the insulation had pulled away from the female end of the cord, creating contact with electricity hazards.     b)  At the folding area - Employees were using a flexible cord (extension cord) to power an iron used for melting plastic shipping material, where the insulation had pulled away from the female end of the cord, creating contact with electricity hazards.    NOTE: IN ADDITION TO ABTEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19
