29 CFR 1926.651(e): Employee was not prohibited to be underneath loads handled by lifting or digging equipment: Excavation site located at the corner of 9800 W. Reno, Oklahoma City OK: For the work conducted on or about September 3, 2014 and at times prior thereto, an employee was observed working in a trench where gravel was being dumped into the trench by a track hoe. The employee was exposed to struck-by hazard while working under the load.

29 CFR 1926.652(b): The slope and configurations of sloping and benching systems constructed by the employer was not in accordance with the requirements of paragraph (b)(1); or, in the alternative, paragraph (b)(2). The configuration was not in accordance with 1926 Subpart P Appendix B figure B-1.2 for type B soils and or Figure B-1.3 for type C soils, in that the support or shield system did not extend the minimum (18) eighteen inches above the top of the vertical side: Excavation site located at the corner of 9800 W. Reno, Oklahoma City OK: For the work conducted on or about September 3, 2014 and at times prior thereto, employees were working in a 12 feet deep excavation with a shield system installed. The top of the shield varied between 12 and 24 inches below the vertical side of the excavation leaving collapsible soil above the protective system exposing employees to struck by and crushing hazards.
