On or about August 25, 2016, no protective devices were in place to protect employees while working in a trench at a depth greater than five (5) feet.  Employees were exposed to the hazards of being crushed by falling soil.       McKee Utility Contractors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1065313, citation number 01, item number 01 and was affirmed as a final order on 09/28/2015, with respect to a workplace located at 1816 W Lindsey.
