29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.      a) 789 St. Georges Ave., Woodbridge, NJ 07095. The employer allowed employees to dry cut masonry block without wearing protective equipment, such as but not limited to, safety glasses, face shields or dust masks, to protect the employee from struck by or inhalation hazards.         Condition occurred on or about September 5, 2014.    b) 789 St. Georges Ave., Woodbridge, NJ 07095. 29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as follows:    a) 789 St. Georges Ave., Woodbridge, NJ 07095.  Employees were allowed to perform exterior stone work operations from a fabricated frame scaffold approximately 13 feet to the ground below that was not fully planked from the front upright to the rear upright.    Condition was observed on or about September 5, 2014.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a) 789 St. Georges Ave., Woodbridge, NJ.  Employees were allowed to use a ladder that was not secured and did not extend 3 feet above the working surface to allow for safe access to the platform of the scaffold.        Condition was observed on or about September 5, 2014.

The employer allowed employees to be exposed to tripping hazards as they stepped from the bottom rungs of the ladder down onto the ground surface that was obstructed by bricks and other items of debris.            Condition was observed on or about 09/05/2014.

Mr. Concrete Corp., was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1), which was contained in OSHA inspection number_313982217, citation number 1, item number 1 and was affirmed as a final order on (10/05/2010), with respect to a workplace located at 2 Park Square, Rahway, NJ 07065.
