29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a)  At the East Corner of the Building - On or about, August 19, 2014, an employee, handing metal roofing material up to the northeast side of the roof, was not wearing a hard hat while being exposed to a possible head injury from impact with objects falling from the workers above him.      No abatement certification or documentation required.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems:    a)  At the East Corner of the Building - On or about August 19, 2014, on a 2:12 pitch roof, an employee, measuring roof structures on the southeast side, was not protected from a fall hazard of 9.2 feet by the use of a guardrail system, safety net system, or personal fall arrest system.      b)  At the East Corner of the Building - On or about August 19, 2014, on a 2:12 pitch roof, two employees, installing metal roofing on the northeast side, were not protected from a fall hazard of 10.15 feet by the use of a guardrail system, safety net system, or personal fall arrest systems.     No abatement certification or documentation required.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a)  At the East Corner of the Building - On or about, August 19, 2014, employees, installing metal roofing material, were not adequately trained to recognize the nature of the fall hazard of 10.15 feet from a 2:12 pitch roof to which they were exposed without the use of a fall protection system.       Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a)  At the East Corner of the Building - On or about August 19, 2014, the ladder used to access and egress the southeast edge of the 2:12 pitch roof did not extend 3 feet above the 9.2-foot eave.      No abatement certification or documentation required.
