29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, or personal fall arrest system:     a) 1727 Ramich Road, Temple, PA / Garage Roof Area; Employees were installing roofing shingles on a 4/12 pitch roof 8 feet 4 inches above the ground without the use of fall protection, on or about 09/15/2015.    b) 1727 Ramich Road, Temple, PA / Shed Roof Area; Employees were installing roofing shingles on a 4/12 pitch roof 8 feet above the ground without the use of fall protection, on or about 09/15/2015.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:      a) 1727 Ramich Road, Temple, PA; Employees were using a Werner 16 Foot Extension Ladder that was not placed 3 feet above the landing to access the roof work area, on or about 09/15/2015.     Advanced Construction Solutions, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 993637, citation number 1, item number 3 and was affirmed as a final order on 04/02/2015, with respect to a workplace located at 875 Berkshire Blvd Wyomissing, PA 19610.     Abatement certification and documentation required within 10 days after abatement date.
