29 CFR 1910.147(f)(2)(i): Whenever outside servicing personnel are to be engaged in activities covered by the scope and application of this standard, the on-site employer and the outside employer shall inform each other of their respective lockout or tagout procedures:    7630 L St., Omaha, NE 68127 (Site-Wide):  The employer is failing to ensure that before work was being performed on the cart tipper, that the exchange of lockout procedures for the cart tipper was completed between them and the subcontractor. The energy of gravity was not identified nor restrained by means such as blocking. The cart tipper fell, injuring two subcontractor employees.      Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed.
