29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) Employees operating a RAS 61.31 folding machine located in the sheet metal shop were exposed to the point of operation between the clamping beam and the work table without any form of guarding. On or about 9/5/14, an employee was injured when his thumb tips were amputated after being crushed between the clamping beam and the work table. (b) Employees operating a ROTO-DIE hydraulic bender in the sheet metal shop were exposed to the point of operation between the dies without any form of guarding.
