On or about June 10, 2024, the employer did not provide the required injury and illness description on the OSHA Form 300 or equivalent for the calendar year 2024.   a ) On or about January 23, 2025, while a Service Technician was performing roof work, the employee sustained frostbite on both hands due to cold weather (Case log Number 1-25), which resulted in a job transfer or restriction case. The Employer's OSHA 300 Form or equivalent for the calendar year 2025 did not contain a complete description of the object/substance that directly injured or made the person ill.
