You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year. Crafts, Inc. - 3403 Menasha Avenue, Manitowoc, Wisconsin. On or about June 10, 2024, the employer did not provide the required injury and illness description on the OSHA Form 300 or equivalent for the calendar year 2024. a ) On or about January 23, 2025, while a Service Technician was performing roof work, the employee sustained frostbite on both hands due to cold weather (Case log Number 1-25), which resulted in a job transfer or restriction case. The Employer's OSHA 300 Form or equivalent for the calendar year 2025 did not contain a complete description of the object/substance that directly injured or made the person ill.
