29 CFR 1904.29(b)(1): A log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in the detail as required by the regulation: U.S. DOT at Suisun Bay Fleet Reserve: The employer did not properly classify the following workplace injuries and illnesses as Days Away From Work (Column H), Job Transfer/Restriction (Column I), or Other Recordable Cases (Column J) on the 2014 OSHA 300 Log; A. On May 25, 2014, a Small Craft Operator received a spider bite on his back. The Supervisors Incident Investigation Report indicated no lost time. B. On May 31, 2014, a Patrol Small Craft Operator experienced lower back pain when picking up a mooring line which resulted in lost time. C. On June 11, 2014, a Security Guard experienced smoke inhalation and has spent several months on job transfer/restriction. D. On June 24, 2014, a Laborer tripped getting on a transportation boat. The Supervisors Incident Investigation Report indicated no lost time. E. On August 6, 2014, a Rigger Worker received a laceration opening a package. The Supervisors Incident Investigation Report indicated no lost time. F. On August 18, 2014, a Laborer jammed his neck on a bulk head. The Supervisors Incident Investigation Report indicated no lost time.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: Suisun Bay Fleet Reserve: The employer did not ensure that all employees, including but not limited to security guards, were provided with hazard communication training to facilitate understanding of the new globally harmonized system, the new standard requires that workers be trained by December 1, 2013 on the new label elements and safety data sheet format, in addition to the current training requirements.
