29 CFR 1926.403(b)(1)(i): Safety of equipment was not determined on the basis of suitability of the equipment for an identified purpose as evidenced by listing, labeling, or certification for that identified purpose: a. On or about September 9, 2014 and prior thereto at a jobsite located at 9005 Gracie Lane, Fort Smith, AR, two metal electrical boxes with knockouts were attached to a 2X6 board lying on the ground. The equipment was not used according to the listing and labeling. Employees plugging cords into the outlet boxes and using equipment such as saws were exposed to the hazard of contacting electrical parts if the knockouts were not in place. b. On or about September 9, 2014 and prior thereto at a jobsite located at 9005 Gracie Lane, Fort Smith, AR, an appliance outlet and cord set was used with plastic insulated wire to supply power to two metal electrical boxes attached to a 2X6 board lying on the ground. The equipment was not used according to the listing and labeling. Employees plugging cords into the outlet boxes and using equipment such as saws were exposed to the hazard of electrical shock and burns.
