29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:         On or about September 10, 2014 at a jobsite located at8507 Silverstone Court, Fort Smith, AR, employees performing work on the ground level such as cutting and handing up material were not wearing head protection, exposing them to the hazard of being struck by falling objects such as hammers and boards being used by employees on the roof and work platform above.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:    On or about September 10, 2014 at a jobsite located at8507 Silverstone Court, Fort Smith, AR, employees used a work platform supported by a GEHL Dynalift 553 rough terrain forklift with telescopic boom to install cedar siding on the gables at the front of the house.   The GEHL 553 Dynalift telescopic boom forklift operators manual prohibited the use of the equipment as a lift for personnel

29 CFR 1926.451(c)(2)(v): The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:    On or about September 10,, 2014 at a jobsite located at8507 Silverstone Court, Fort Smith, AR, the work platform supported by a GEHL Dynalift 553 rough terrain forklift was not attached to the forks.  Employees standing on the platform to install cedar siding on the gables at the front of the house were exposed to the hazard of falls to the ground below if the platform was to come off of the forks or move.
