29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  (a) The employer did not initiate and maintain an accident prevention program to ensure employees were able to recognize and avoid unsafe conditions and were knowledgeable on the methods to control or eliminate hazards associated with their work environment, such as, but not limited to, working from scaffold systems, using pneumatic nail guns and operating electrical powered tools. The employees were exposed to struck-by, electrical and fall hazards.

29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide:    (a) On the north side of the residence, a scaffold system with a 10-inch wide work platform was being used by employees to install siding on a 2-story single family residence. The employees were exposed to a fall of approximately 19-feet to the next lower level.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:   (a) On the north side of the residence, the employer did not provide a safe means of access for employees using a pump jack scaffold to install siding on a two-story single family home, exposing the employees to a fall of approximately 19-feet to the next lower level.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  (a) On the north side of the residence, employees were not protected from falling while installing siding from a pump jack scaffold on a 2-story residence without the use of a fall protection system. The employees were exposed to a fall of approximately 19-feet to the next lower level.

Employees were installing siding on a 2-story residence, exposed to a fall of approximately 19-feet to the next lower level.
