29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: (a) VA Medical Center, Memphis, TN - On or about 9/9/14, employees were exposed to slip and trip hazards when the work area was not kept free of debris and equipment. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.150(a)(2): Access to all available firefighting equipment was not maintained at all times: (a) VA Medical Center, Memphis, TN - On or about 9/9/14, employees were exposed to burn hazards when fire fighting equipment, such as a fire extinguisher and fire alarm pull, were blocked preventing access to the equipment. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
