OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards:   a) On or about 9/10/2014, Generator Room:  Employees were exposed to falls of up to twelve feet from an open sided second floor area.    b) On or about 9/10/2014, Second Floor Eating Area:  Employees were exposed to falls of up to ten feet from an unguarded stairway opening         c) On or about 9/10/2014, Second Floor Eating Area:  Employees were exposed to falls of up to ten feet from an open sided floor area used as a loading location.     d) On or about 9/10/2014, Second Floor Eating Area:  Employees were exposed to falls of up to ten feet down a staircase due to a lack of a hand rail.    e) On or about 9/10/2014, Main Office Area:  Employees were exposed to trip falls due to an unguarded floor hole in the concrete.   f) On or about 9/10/2014, Second Floor Locker Area:  Employees were exposed to trip falls due to an unguarded floor hole in the plywood.   g) On or about 9/10/2014, Milk Storage Area:  Employees were exposed to trip falls due to an unguarded floor opening inside of the access door from the chemical room.    h) On or about 9/10/2014, Manure Digester Area:  Employees were exposed to falls of eight feet into Weir's.   Among other feasible and acceptable means of abatement would be to install top rails and midrails on the open sided floors, stair railings on stair cases with four or more risers, and covers over floor holes to comply with the American National Standards Institute, ANSI, Standard A1264.1-2007, Safety Requirements for Floor and Wall Openings, Railings, and Toeboards, Sections 3, 5 and 7.   Abatement certification is required for this item.

OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to chemical hazards:   a) On or about 9/10/2014, Throughout the Farm:  Employees were dispensing and using corrosive chemicals including, but not limited to Formaldehyde, Sodium Hypochlorite Solution, Longline CIP, and Hi-Clean while performing cleaning operations and replenishing foot baths. Employees exposed to splash hazards.  The employees were not provided with or using any eye and face protection, and were not provided with any eye wash facilities.    Among other feasible and acceptable means of abatement would be to provide eyewash facilities that will provide 15 minutes of continuous flow of water and will comply with the  American National Standards Institute, ANSI, Standard Z87.1 - 2003 Occupational and Educational Personal Eye and Face Protection Devices Section 6, and the American National Standards Institute, ANSI, Standard Z358.1- 1998 Emergency Eyewash and Shower Equipment Sections 6 and 7 .    Abatement certification is required for this item.

OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to asphyxiation or atmospheric hazards:   a) On or about 9/10/2014, Throughout the Farm:  Employees were required to enter into various confined spaces such as, but not limited to the Weirs and pits associated with the Manure Digester, and pits associated with the Manure collection system in order to open and close valves and clear blockages.  Employees were exposed to atmosphere that were immediately dangerous to life and health.   Among other feasible and acceptable means of abatement would be to identify and evaluated all of the potential confined spaces, implement a Confined Space Program which would include identifying and labeling confined spaces, performing atmospheric testing and monitoring, implementation of entry permits, ventilation, retrieval system, contractor entry and employee training.  Also, comply with the  American National Standards Institute, ANSI/ASSE Z117.1, Safety Requirements for Confined Spaces, Sections 3 through 17.   Abatement certification is required for this item.

OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to electrocution hazards:     a) On or about 9/10/2014, In the Milk Room:  A metal cased pump was not provided with a continuous path to ground.   b) On or about 9/10/2014, In the Milk Room:  An outlet intended for a wet location was not provided with a cover that would stay closed.     c) On or about 9/10/2014, Second Floor, Bathroom/ Employee Area:  A panel box had exposed energized parts.    d) On or about 9/10/2014, Second Floor, Bathroom/ Employee Area:  Outlets were not provided with covers, and had exposed energized parts.     e) On or about 9/10/2014, Second Floor, Bathroom/ Employee Area:  A light fixture was not secured, and was suspended by Romex.    f) On or about 9/10/2014, Second Floor, Bathroom/ Employee Area:  Energized Romex cable was crushed where it entered in the frame of metal cased unit.   g) On or about 9/10/2014, Milking Parlor Area:  A light fixture had exposed energized parts.  h) On or about 9/10/2014, Milking Parlor Area:  A section of conduit was pulled apart, exposing and chafing wiring.  i)  On or about 9/10/2014, Milking Parlor Area:  A fan had its power cord pulled from its frame so that there was no longer any strain relief.   Among other feasible and acceptable means of abatement would be to comply with the National Fire Protection Association (NFPA) Code 70, National Electric Code, Articles 110 and 250.   Abatement certification is required for this item.
