(c) On September 11, 2014, at worksite 301 McCleary, Excelsior Springs, Missouri.-  in the Loading Dock area, employees were exposed to electrical hazards created when Hubbell brand Dock lights that was listed and labeled for 3 prong connection were intentionally cut off by maintenance personnel and feed directly in powered electrical junction boxes.   (d) On September 11, 2014, at worksite 301 McCleary, Excelsior Springs, Missouri.-  in the Loading Dock area, Production area and storage area, employees were exposed to electrical hazards created when Fluorescent Lighting fixtures listed and labeled for 3 prong electrical connection were intentionally cut off by maintenance personnel and feed directly in powered electrical junction boxes.
