Schwarz Partners Packaging, LLC dba The Royal Group - Marion, Ohio: On or about September 2, 2014, the employer did not provide employees with stairs for parts of the WardJet water jet machine (s/n 0614-Z131) which requires access for setup.  Employees are exposed to slip/fall hazards when climbing on the machine's frame to gain access to the table and cutting heads.

The machine was not locked out when an employee accessed the table to set-up the position of the cutting heads.
