OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards while operating a forklift at the loading dock:         a. On or about October 16, 2017 the employer did not ensure employees were protected from fall hazards while loading straight box trucks. The forklift operator was unloading a small straight box truck at the dock when the box truck began to roll away from the dock, which resulted in the forklift and the powered industrial truck operator becoming wedged between the truck and the loading dock.      Among other methods, the following are feasible and acceptable methods to correct the hazard.  1. Install a dock lock system appropriate to small box straight trucks and train employees in the proper operation of the system as required in the manufacturer owners manual.
