Employees throughout the facility were thereby exposed to struck-by hazards.

29 CFR 1910.178(o)(1): Only stable or safely arranged loads shall be handled. Caution shall be exercised when handling off-center loads which cannot be centered. a) Warehouse - On or about September 15, 2014, a powered industrial vehicle (PIV) operator was allowed to use a (PIV) to slide 90 pallets ( ten stacks of pallets each nine high) simultaneously along the warehouse floor. The employer did not ensure that only stable or safely arranged loads were handled or moved. Employees throughout the facility were thereby exposed to struck-by hazards.
