Employees were thereby exposed to the hazards associated with the unexpected energization of the equipment.

29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware shall be provided by the employer for isolating, securing or blocking of machines or equipment from energy sources. Production area - On or about August 30, 2016, band saw operators were required to perform tasks on Smart pallet dismantlers including, but not limited to changing blades and cleaning the Smart pallet dismantler saws. The employer did nor ensure the employees were provided locks or other hardware to isolate energy sources prior to performing the tasks. Employees were thereby exposed to caught-in hazards.

Production area/.southwest - On or About August 30, 2016, band saw operators were required to perform tasks on the two Smart pallet dismantler bandsaws, including, but not limited to changing and cleaning the bandsaws. The employer did not ensure that each employee received training as an authorized employee prior to performing the tasks. Employee were thereby exposed to caught in hazards.
