29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Pug mill, Altoona Building, 500 Mill Street, Freeport PA 16229: Observed on or about September 11, 2014, the rotating auger on a Chambers 12C pug tub was not guarded. The auger was located in a tub that was open on top. The opening measured approximately 2 feet 4 inches wide and 11 feet 5 inches long. Pug mill operators working between the pug mill and the pug mill control panel were exposed to caught-by and cut-by hazards.

29 CFR 1910.219(f)(1): Gear(s) were not guarded by a complete enclosure or by one of the methods specified in 29 CFR 1910.219(f)(1)(ii) and (f)(1)(iii): a) #1 Dry Press, Production area next to kiln, 500 Mill Street, Freeport PA 16229: Observed on or about September 11, 2014, gears located to the left of the 400 ton #1 press were not guarded. A gap of 1 feet 9 inches between the left side of the #1 dry press and the wall permitted employee access to the area of the gears. Employees were exposed to caught-by hazards while operating the #1 dry press.

The lamp was located approximately 6 feet 4.5 inches from the floor. Employees operating the #1 Dry Press were exposed to cut-by and electrical shock hazards.
