29 CFR 1910.215(a)(1): Abrasive wheel(s) used on grinding machinery were not used with safety guard(s):      a)  Press area, 500 Mill Street, Freeport, PA 16229:  Observed on or about June 29, 2015, the right abrasive wheel on a Dewalt DW758 bench grinder was not equipped with a tongue guard.  A gap of approximately 1 and 1/8 inches existed between the right grinding wheel and the frame of the machine.  Employees using the bench grinder where exposed to struck-by and fire hazards while using the bench grinder.                                                        b)  Press area, 500 Mill Street, Freeport, PA 16229:  Observed on or about June 29, 2015, the left abrasive wheel on a Dewalt DW758 bench grinder was not equipped with a tongue guard.  A gap of approximately 1 and 1/2 inches existed between the left grinding wheel and the frame of the machine.  Employees using the bench grinder where exposed to struck-by and fire hazards while operating the bench grinder.

29 CFR 1910.215(a)(4):  Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:   a)  Press area, 500 Mill Street, Freeport, PA 16229:  Observed on or about June 29, 2015, the work rest on the left wheel of a Dewalt DW758 bench grinder was not adjusted such that the maximum opening between the abrasive wheel and the work rest was not more than 1/8 of an inch.  The gap between the abrasive wheel and the work rest was approximately 3/4 of an inch.  Employees using the bench grinder were exposed to caught-by hazards.  b)  Press area, 500 Mill Street, Freeport, PA 16229:  Observed on or about June 29, 2015, the work rest on the right wheel of a Dewalt DW758 bench grinder was not adjusted such that the maximum opening between the abrasive wheel and the work rest was not more than 1/8 of an inch.  The gap between the abrasive wheel and the work rest was approximately 7/16 of an inch.  Employees using the bench grinder were exposed to caught-by hazards.

Employees who were not trained as authorized employees under the employer's energy control program were assigned maintenance tasks, including die changes, on the #1, #3, and Y presses.

29 CFR 1910.303(g)(1)(ii):  Working space about electric equipment rated 600 volts, nominal, or less was used for storage:   a) Binding area, 500 Mill Street, Freeport, PA 16229:  Observed on or about June 29, 2015, the working space in front of electrical equipment (480 volt, 3 phase) located on a wall in the binding area was used storage.  A table, boxes, and a fan were stored in front of the electrical equipment.  Employees working in the main shop were exposed to fire and electric shock hazards created by restricted access to the electrical panels.

29 CFR 1910.305(b)(1)(ii): Unused openings in cabinets, boxes, and fittings were not effectively closed:   a)  Press area, 500 Mill Street, Freeport, PA 16229:  Observed on or about September 02, 2015, unused openings on the side and on the front cover of a Square D brand distribution panel (120/240 volt) labeled 'PO-2" were not effectively closed.  The openings could permit the accumulation of dust and debris inside the cabinet.
