29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about 9/18/2014, at the above addressed jobsite, an employee who was not wearing head protection was exposed to a struck-by hazard when working in close proximity to an excavator bucket that was being used to lift and position a concrete drainage pipe.
