29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access: (a) 1242 Main St. (RT 159), Agawam, MA: On or about September 18, 2014, an employee was observed using an extension ladder to access the roof of this residence that did not extend any distance past that upper landing surface. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system: (a) 1242 Main St. (RT 159), Agawam, MA: On or about September 18, 2014, two employees were observed roofing this residence without any fall protection more than 18 feet above the ground below. Fitzgerald Roofing was previously cited for a repeat violation of this occupational safety and health standard 29 CFR 1926. 501 B (13) which was contained in OSHA inspection number 760042 citation number 2, item number 1 and was affirmed as a final order on January 8, 2013, with respect to a workplace located at 644 Armory St., Springfield,MA. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
