29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: Worksite located at 13930 S Pennsylvania Ave. Oklahoma City, OK 73170. On or about September 18, 2014, and at times prior thereto, adequate protective systems, such as sloping, benching, or shielding were not used to protect employees while working in a trench at a depth of approximately 10 feet. The employees were exposed to engulfment or crushed-by hazards from collapsing soil.
