29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about October 02, 2018, no protective systems were in place to protect employees working in a trench that was more than 5 feet in depth, exposing the employees to the hazard of being crushed by falling soil. Mid South Contracting LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1202478, citation number 01, item number 01 and was affirmed as a final order on April 14, 2017, with respect to a workplace located at 17804 Silverhawk Way, Edmond, OK 73034. And Mid South Contracting LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 995884, citation number 01, item number 01 and was affirmed as a final order on December 19, 2014, with respect to a workplace located at 1390 S Pennsylvania Ave. Oklahoma City, OK 73170.
