29 CFR 1926.501(b)(15): Except as provided in 1926.500(a)(2) or in 1926.501 (b)(1) through (b)(14), each employee on a walking/working surface 6 feet (1.8 m) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system: a) Job site, 1000 Noble Energy Drive, Canonsburg, PA: On or about September 16, 2014, during construction work, the employer did not ensure that appropriate fall protection systems/devices were used while an employee was working up in the piping near the ceiling to weld pipe sections and was exposed to a fall hazard of approximately 10 feet.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder: a) Job site, 1000 Noble Energy Drive, Canonsburg, PA: On or about September 16, 2014, during construction work, the employer did not ensure that the ladder used to gain access to the pipes near the ceiling was extended 3 feet above the landing surface or that the top of the ladder was secured and a grab rail was provided, exposing an employee to a fall hazard of approximately 10 feet.
