29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: (Construction Reference: 1926.59)    On or about September 22, 2014, at the worksite located at 17625 Chenal Parkway in Little Rock, Arkansas, a hazard communication program had not been developed for the benefit of employees using and/or exposed to hazardous chemicals/ingredients such as but not limited to crystalline silica and calcium oxide contained in the Ash Grove Type S pro mix masonry mortar being mixed and used by employees performing cast stone masonry application tasks.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):    On or about September 22, 2014, at the worksite located at 17625 Chenal Parkway in Little Rock, Arkansas, on the supported scaffold erected at the northwest corner of the tower, the 3rd level platform that employees accessed to perform cast stone masonry tasks, was not decked the full width of the scaffold.  This condition exposed employees to falls from elevations.

29 CFR 1926.451(c)(2)(i): Footings were not level, sound, rigid and capable of supporting the loaded scaffold without settling or displacement:    On or about September 22, 2014, at the worksite located at 17625 Chenal Parkway in Little Rock, Arkansas, on the supported scaffold located at the northwest corner of the tower, various sizes of unsecured stacked wooden boards and hollow core masonry block were used to support scaffold system.  This condition exposed employees to falls from elevation and/or a scaffold collapse.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:    On or about September 22, 2014, at the worksite located at 17625 Chenal Parkway in Little Rock, Arkansas, northwest corner of the tower, a safe access was not provided for employees to access supported scaffold work platforms.    (a)  Employees accessed and dismounted the 2nd and 3rd level work platforms by climbing down the scaffold end frames.    (b)  Employee dismounted from the 2nd level work platform and climbed down the cross braces to lower to ground level.    These conditions exposed employees to slips and falls from elevations.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    On or about September 22, 2014, at the worksite located at 17625 Chenal Parkway in Little Rock, Arkansas, on the supported scaffold erected at the northwest corner of the tower, guardrails were not provided on all open sides and ends of the 2nd and 3rds level work platforms to protect employees accessing platforms and engaged in cast stone masonry tasks.  Employees were exposed to a scaffold collapse and/or falls from elevation.
