The Andersons, Inc. located in Maumee, Ohio: On or about September 23, 2014, the employer did not ensure the Fisher back flow check valves located on the anhydrous ammonia process piping complied with recognized and generally accepted good engineering practices. c. The Andersons, Inc. located in Maumee, Ohio: On or about September 23, 2014, the employer did not ensure the Fisher excessive flow valves located inside of the anhydrous ammonia storage tank complied with recognized and generally accepted good engineering practices. d. The Andersons, Inc. located in Maumee, Ohio: On or about September 23, 2014, the employer did not ensure inspection and maintenance procedures were established and implemented to maintain the on-going integrity of the Fisher and Squibb Taylor globe valves located on anhydrous ammonia process lines. c. The Andersons, Inc. located in Maumee, Ohio: On or about September 23, 2014, the employer did not ensure inspection and maintenance procedures were established and implemented to maintain the on-going integrity of the anhydrous ammonia process piping. d.
