29 CFR 1910.94(a)(5)(ii): Abrasive-blasting respirators were not worn by all abrasive-blasting operators when using silica sand in manual blasting operations where the nozzle and blast were not physically separated from the operator in an exhaust ventilated enclosure. (a) Located outside building 6, the employee engaged in sand blasting operation on a cab assembly was not provided a respirator that protected the employee's head, neck, and shoulders from rebounding abrasive material.

29 CFR 1910.132(c): All personal protective equipment was not of safe design and construction for the work performed: (a) Located in building 6, the wire rope clamps installed on the horizontal life line as part of a fall protection system were not installed per the manufacturer's recommendation and subject the wire rope to damage exposing employees to potential fall hazards. (b) Located in building 6, the make shift anchor point device attached to the overhead steel beam was not capable of sustaining the dynamic load(s) imposed during a fall arrest event and exposing employees to potential fall hazards. (c) Located in building 6, the self-retracting lifeline (Model No. AD111A, S/N: 333332) was used in conjunction with the Titan by Miller Harness/Lanyard combo (Model: TK4051/U/6FTAKU, S/N: 170576) as part of a fall arrest system which was not being used in accordance with the manufacturer's recommendations exposing employees to potential fall hazards.
