On or about and at times prior to June 17, 2025, at the establishment located at 1176 Coon Street, Rhinelander, Wisconsin; the employer did not ensure that machines were effectively guarded in the following instances: a) The Uni-Hydro PRO 56 Ton Ironworker (Model: P56-14, Serial: 1P7749X) Punch Table and Adjustable Jig guard was removed, exposing employees to amputation hazards. b) The Uni-Hydro PRO 56 Ton Ironworker (Model: P56-14, Serial: 1P7749X) Notching Station was not guarded, exposing employees to amputation hazards.
