29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:      a) Employees were exposed to head injury while working without protective helmets in an area below carpentry and hoisting  operations where lumber was being delivered to an upper deck of the building. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):               a) Employees were exposed to the hazard of falling while performing carpentry and hoisting work from first story deck surfaces without the use of fall protection.       b) Employees were exposed to the hazard of falling while performing carpentry from second story deck surfaces and top plates without the use of fall protection..       In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     a) Employees were exposed to the hazard of falling from an elevation of approximately 12 feet where a portable extension ladder provided for second story access and egress did not extend at least 3 feet above the landing surface of the second story deck.
