29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:         a) On or about April 20, 2017, employees were exposed to eye injury while operating pneumatic nail guns without the eye protection during carpentry work. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):               a) Top Plate Above Garage - On or about April 20, 2017, employees were exposed to the hazard of falling between 8 and 19 feet while performing carpentry work.  b) Front Yard Window/Wall Opening - On or about April 20, 2017, employees were exposed to the hazard of falling approximately 14 feet while performing carpentry work.  c) Front Yard Top Plate - On or about April 20, 2017, employees were exposed to the hazard of falling approximately 19 feet while performing carpentry work.  d) Rear Yard Window/Wall Opening - On or about April 20, 2017, employees were exposed to the hazard of falling between 6 and 14 feet while performing carpentry work.  e) Second Story Interior - On or about April 20, 2017, employees were exposed to the hazard of falling approximately 9 feet while performing carpentry work.    M & J Construction Service, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 996632, citation 1, item 3, and was affirmed as a final order on October 15, 2014, with respect to a worksite located at Sunset Lot #8, Barrington, Illinois          In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
